About the role
Western Health is seeking an experienced and motivated Team Leader to join our new and innovative Western Health Mental Health at Home (WHMH at Home) service — an exciting model of care delivering acute mental health treatment in the home environment.
WHMH at Home is a 24/7 service providing the equivalent of ten acute mental health beds in the community. Underpinned by the Hospital in the Home (HITH) model of care, it provides intensive, recovery-oriented support to consumers experiencing acute mental health crises who would otherwise require hospital admission.
As Team Leader, you will play a pivotal leadership role in coordinating the day-to-day delivery of high-quality, person-centred care. Working closely with the Program Manager, you will provide clinical leadership, guidance, and support to the multidisciplinary team while promoting excellence in safety, quality, and recovery-oriented practice.
