THE ROLE
The Team Leader – Case Management will be required to manage a team of case managers in their work with RMP clients. This involves developing and monitoring workers in assessment and planning as per the RMP Case Management Framework.
The Team Leader is also an active member of the RMP Operations Team and will work with program developments and changes. This will include identifying service system gaps and operational developments within a solution focused framework.
ACCOUNTABILITIES INCLUDE
- Leadership - Build a cohesive team with clarity around goals and accountabilities; obtain needed personnel, resources and information for the team; support individuals and the team, delegate responsibilities appropriately; bring together the best possible group to achieve objectives.
- Change Management - Identify the need to change; describe the reasons for the change; actively promote and manage change; remain calm and optimistic, even when things don't go as planned; draw upon a range of sources for ideas and solutions.
- Accountability - ensure the team achieves its goals, meets expectations, and upholds team values. Take ownership of the team's actions, outcomes, and impact on the organization. Foster a culture of accountability by setting clear expectations, providing support, and holding themselves and their team members responsible for their contributions
- Quality Improvement -Drive quality improvement by fostering a culture of continuous improvement, providing support and resources, and actively participate in improvement efforts. Set clear expectations, encouraging feedback, and recognizing achievements.
