About Health Independence Program
Austin’s Health Independence Program (HIP) is funded by the Victorian Department of Health and Human Services and is part of the Continuing Care Division. HIP provides multi-disciplinary services that support people with health problems and other complex conditions to optimise their function and self-management skills so that they can live independently and well in their homes and community. The Health Independence Program encompasses many of the services that deliver health care to support the transition from hospital to home. These services can also prevent the need for a hospital presentation or stay, with people able to access these services directly from the community. HIP works closely with other such services within the Continuing Care Division including Better@home; Residential In Reach and Transition Care Program.
About the role
Reporting to the HIP Specialist Services Manager, the key function of the Team Leader role is to coordinate the daily operations of HIP Rehabilitation Services (home and centre based services) across the 3 Austin Health sites. This is achieved as part of a job-share arrangement with the other part-time Community Rehabilitation Services Team Leader.
The HIP Rehabilitation Services Team Leader will facilitate an interdisciplinary approach to client care ensuring that key service performance deliverables are met including consumer outcomes, activity targets, human resource and quality. The HIP Rehabilitation Services Team Leader will also have a clinical role within the team and will travel between Austin campuses as required.
