Role description
Dhelkaya Health is seeking a motivated and values-driven Support at Home Care Partner to join our growing community services team. In this rewarding role, you will coordinate holistic, person-centred supports that enable older people to remain safe, independent and connected in their own homes. Working within the new Support at Home program framework, you will undertake comprehensive assessments, develop goal-directed care plans, manage participant budgets, and collaborate with families, allied health professionals and community partners to deliver high-quality, compliant services. This is an opportunity to contribute to meaningful aged care reform while building trusted relationships and making a genuine difference in the lives of older people across our community.
As a Care Partner, you will work directly with older people to assess needs, co-design personalised support plans, and monitor care delivery. This role requires knowledge of aged care principles, wellness and reablement approaches, and a strong commitment to quality and accountability as per the Aged Care Quality Standards.
Key Responsibilities
Ideal candidates will have experience in aged care case management and have an understanding of consumer-directed care models.
