The Stroke Coordinator at Northeast Health Wangaratta plays a vital role in ensuring stroke patients receive seamless, high-quality care from the moment they present to the Emergency Department through to recovery in the community. This position is responsible for coordinating services across acute inpatient wards, supporting timely transfers to subacute and rehabilitation facilities, and guiding safe, well-planned discharges. By working closely with multidisciplinary teams, patients, and families, the Stroke Coordinator helps deliver consistent, patient-centred care that improves outcomes and supports recovery at every stage of the stroke journey.
