2 x Senior Sentinel Events Officer (761554 & 761556)
The Senior Sentinel Events Officer is a vital member of the Review and System Learning Unit within the Safety Branch at Safer Care Victoria (SCV). This role supports the delivery of SCV's sentinel event program.
Reporting to the Manager, Sentinel Events and Safety Reviews, the Senior Sentinel Events Officer assists health services with the planning, execution, and documentation of sentinel event reviews. Drawing on principles of systems thinking, human factors, and safety science, the role plays a critical part in supporting health services with review methodology and generating insights that inform practical and sustainable safety improvements across the Victorian health system.
The Senior Sentinel Events Officer uses systems and surveillance to identify learnings and themes in sentinel event reviews, analysing data, synthesising findings, and supporting the development of recommendations
The role ensures reviews are conducted in a timely, respectful, and methodologically rigorous manner, with outputs aligned to SCV's values of transparency, learning, and collaboration.
Working as part of a multidisciplinary team, the Senior Officer engages with health services and internal stakeholders to foster a culture of system learning and continuous improvement. They also support sector capability building in incident review and risk mitigation through contributions to tools, resources, and process improvements.
This role supports SCV's overarching goal of reducing harm and enhancing the safety and quality of care in Victoria, contributing to a psychologically safe and inclusive team environment that values the voices of patients, families, and clinicians in driving system learning.
3 x Senior Review Officer (761557, 761558 , 761559)
The Senior Review Officer works as a member of the Review and System Learning Unit within the Safety Branch at Safer Care Victoria (SCV). This role contributes to SCV's system learning and safety improvement agenda by leading the delivery of complex, system-focused reviews that identify contributory factors to harm, highlight emerging safety risks, and generate actionable insights to improve care quality and reliability across the Victorian health system.
As a specialist in human factors, systems thinking, or safety science, the Senior Officer will bring expertise from healthcare or other high-reliability sectors such as defence, aviation, or emergency services. They will play a key role in understanding how complex system conditions contribute to patient safety issues and in translating that understanding into practical, sustainable improvements.
The Senior Review Officer will lead and manage reviews, ensuring they are conducted using contemporary, evidence-informed methodologies that are both timely and thorough. Reviews synthesize insights from multiple sources such as incident data, workforce and consumer experience, system signals, and organisational behaviour to explore clinical, cultural, organisational, or process-related contributors to harm and potential improvements to achieve high quality care. Insights are considered against the Victorian Clinical Governance Framework domains, clinical guidelines, and standards, ensuring a reflection of best practice for achieving high quality care. The review specialist will also monitor and track review insights and implementation of actions by the Health Service and present these to the relevant SCV Governance committee(s).
Working as part of a multidisciplinary team – including experts in safety science, clinical practice, human factors, and data analytics – the Senior Officer will actively support SCV's goal of creating a high-pe rforming, learning health system. They will collaborate with stakeholders across SCV, the Department of Health, and the health sector to ensure that review findings lead to real-world improvements in safety and care delivery.
The Senior Review Officer will report to the Manager, Sentinel Events and Safety Reviews, and contribute to the broader work of the Safety Branch in delivering SCV's strategic objectives, including reducing preventable harm and enhancing the quality, reliability, and person-centredness of care across Victoria
