Senior Emergency Department Care Coordinator
About the Role:
Western Health is seeking a passionate Senior EDCC (Emergency Department Care Coordination) professional to join our innovative, transdisciplinary team!
In this role, you’ll lead and coordinate care in the Emergency Department, working alongside a diverse team of nurses, occupational therapists, physiotherapists, and social workers to prevent avoidable admissions, optimise patient outcomes, and ensure seamless transitions from ED to home.
This role is offered on a permanent, part-time basis at 32 hours per week on a rotating, 7-day roster (AM and PM shifts required).
The service operates between 7:30am and 9:00pm, 7 days a week.
Key Responsibilities:
As a Senior Care Coordinator, you will play a pivotal role in both direct clinical care and the professional supervision of your discipline, while contributing to the strategic direction of the EDCC service.
The ED Care Coordinator will also provide support to the Rapid Allied Health team, by providing discipline specific assessment and intervention in patients’ homes to support safe discharge.
