About the role:
We are currently seeking expressions of interest from experienced Registered Nurses for a Paediatric Hospital in the Home (HITH) Liaison/HITH Nurse (dual role) based at Sunshine Hospital.
Our HITH service continues to grow, with exciting developments including the introduction of a dedicated Paediatric HITH stream. This is a dual/interchangeable position offers a unique opportunity to combine liaison, coordination and direct clinical care, supporting children and their families across the continuum of acute care.
As the Paediatric HITH Liaison Nurse, you will play a key role in identifying and coordinating suitable referrals, undertaking patient assessments, providing clinical advice and telehealth support, coordinating care and ensuring safe, seamless transitions into the HITH service.
As the Paediatric HITH Nurse, you will deliver inpatient-equivalent care in the home through home visits and community outreach. This includes comprehensive clinical assessment, ongoing monitoring, administration of treatments such as intravenous therapies, patient and family education, supporting children to receive safe, high-quality care in the comfort of their own home.
You may also work intermittently within the inpatient paediatric ward to support operational requirements, maintain clinical capability and strengthen partnerships.
