The Royal Melbourne Hospital Mental Health Services provides specialist recovery-focused care for adults and older adults experiencing serious mental illness. Our multidisciplinary teams deliver high quality, trauma informed and person-centred inpatient and community mental health services.
About the Role
The Progression of Care Navigator supports timely discharge planning and consumer flow across the Adult Acute Inpatient Unit. Working closely with multidisciplinary teams and community providers, you will help coordinate safe transitions of care, identify barriers to discharge and support continuity of care for consumers.

You are an experienced mental health nurse with strong organisational skills and a passion for recovery-oriented care.
