Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.
Northeast Health Wangaratta is seeking a dynamic and motivated Program Coordinator to provide day-to-day operational leadership for our Transition Care Program. This pivotal role supports the delivery of high-quality, holistic care that enables clients to maximise their independence, function and quality of life while reducing avoidable hospital admissions and supporting effective patient flow across the organisation.
The Transition Care Program delivers time-limited, goal-directed clinical care and brokered services in both centre-based and home-based settings. The Program Coordinator will lead and support these services to ensure safe, effective and person-centred care that meets the needs of clients during their transition from hospital to home or other care settings.
The successful applicant will demonstrate innovative problem-solving skills, the ability to foster strong collaboration across multidisciplinary teams, departments within Northeast Health Wangaratta and external service providers, and a commitment to excellence in service delivery. This role also plays a key part in promoting Northeast Health Wangaratta as a high-quality regional health service provider and contributing to positive outcomes for clients, staff and the broader community.
