About the role
The Primary Care (PC) intake worker is an entry point for referrals to PC and connects consumers the most appropriate services at Barwon Health (BH) for their health issues. The intake worker has a strong overview of services available across BH and can direct consumers to the most appropriate services as required.
The intake worker, is responsible, under the direction of a relevant health professional (Registered Nurse or Allied Health Practitioner), for the provision of holistic screening of referrals and enquires, in order to achieve access to the correct services and priority for care according to Consumer’s needs. Working within their scope of practice, they will Identify additional services of benefit to the client and provide a range of information to the Consumer about services, fees, eligibility and waiting times.
The intake worker will liaise closely with the team to confirm service prioritisation or escalate care to ensure a safe and high quality experience for our consumer. This role will work in partnership with Information and Access Service staff and site Clinical and Administrative staff.
