Role description
Dhelkaya Health is seeking a motivated and organised Post‑Acute Care (PAC) Coordinator to join our Community Nursing team. This role supports people living in the Mount Alexander, Macedon Ranges and Central Goldfields shires following discharge from hospital.
Reporting to the Community Nursing Unit Manager, the PAC Coordinator plays a vital role in delivering person‑centred, community‑based care during the first month after hospital discharge, helping people to recover safely and regain independence at home.
The Post‑Acute Care Program coordinates short‑term community services to support recovery following a hospital admission. As PAC Coordinator, you will:
