About the Health Independence Program Department
HIP is part of the Continuing Care Division at Austin Health and provides a range of multi-disciplinary supports and services for clients to transition safely from hospital to the community. We support people with health problems and other complex conditions to optimise their function and self-management skills so that they can live independently and well in their homes and community.
About the role
As the Grade 2 HIP Fast Track Care Management Occupational Therapist you will enjoy the support of a multidisciplinary team as well as the other Occupational Therapists working in the Health Independence Program.
As one of two OTs in the team, you will work closely with the complex care managers and physiotherapists to ensure our clients are well supported at home. The OT is an integral member of the team, assessing clients and arranging ongoing equipment and service needs. You will also advise the complex care managers about ongoing support that our clients may need.

As the Occupational Therapist Grade 2, you will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.
