Warragul Communiy Mental Health team are seeking an experienced and innovative Nurse Practitioner – Community Mental Health to provide advanced clinical leadership on a full time ongoing basis. This autonomous and collaborative role combines specialist mental health assessment, diagnosis, treatment, prescribing, and care coordination with a strong focus on improving the physical health outcomes of people living with mental illness. Working closely with consumers, carers, psychiatry, general practice, and multidisciplinary teams, you will lead the Clozapine and Equally Well programs, drive evidence-based practice, and contribute to service development, education, research, and quality improvement initiatives.
