About the role:
Western Health is seeking an experienced and motivated mental health clinician to join our Community Mental Health Team as a Case Manager.
This is an opportunity to provide meaningful, recovery-oriented care to people experiencing complex mental health needs while working within a supportive multidisciplinary team. You will partner with consumers, families, carers and service providers to promote safety, wellbeing, recovery and participation in the community.
Key responsibilities:
As a Community Mental Health Clinician and Case Manager, you will manage an allocated caseload and provide comprehensive assessment, care planning, clinical intervention and service coordination.
