About the role:
The Intensive Community Treatment (ICT) team plays a vital role in supporting older adults to achieve the best possible mental health and wellbeing outcomes in the comfort of their own homes.
As a Mental Health Clinician, you will work as part of a collaborative multidisciplinary team within the Older Persons Mental Health and Well-being Program, providing compassionate, holistic and evidence-based care to people aged 65 and over experiencing an acute episode of mental illness.
You will undertake mental health assessments, develop and implement individualised treatment plans, monitor recovery, and document and communicate clinical updates to ensure coordinated, high-quality care. Building strong relationships with consumers, families, carers and other healthcare professionals will be central to your role, helping to ensure care is responsive, person-centred and recovery focused. Medication supervision and/or administration may also be required.
This is an opportunity to use your clinical expertise and discipline-specific skills to make a real difference in the lives of older people. The ICT team provides an intensive, community-based alternative to hospitalisation where appropriate, supporting consumers to remain at home during acute episodes of illness, promoting recovery and helping to minimise inpatient stays.
Services are delivered seven days a week, between 8:30 am and 5:00 pm, ensuring timely and accessible support when it matters most.
