Role description
Dhelkaya Health is seeking an experienced and motivated At Home Nurse Coordinator to join our dynamic At Home Program, delivering high‑quality, person‑centred clinical care to clients in their homes across the Mount Alexander Shire.
This is a key clinical leadership role supporting innovative models of care that provide safe, effective alternatives to hospital‑based treatment.
Dhelkaya Health's At Home Program provides multidisciplinary clinical care in the home or place of residence, when safe to do so. The program operates 7 days a week and includes:
The program aims to deliver the right care, in the right place, at the right time, improving patient flow, consumer experience, and health outcomes.
The At Home Nurse Coordinator is a Clinical Nurse Consultant role responsible for delivering expert nursing care and coordinating At Home services across all phases of care. You will advocate for clients, support their health goals, and ensure coordinated, high‑quality multidisciplinary care.
This role plays a pivotal leadership function, overseeing referral screening, triage, intake, assessment, care planning, review, transition, and discharge processes. You will also have a key role in recognising and responding to acute deterioration and supporting safe transitions to other services when required.
