Role Description
Are you passionate about delivering high-quality, person-centred support services in the community? We're looking for a dynamic and organised Home Care Coordinator to oversee the day-to-day delivery of our Community and In-home Support programs, including Home Support Workers and Social Support services.
As the Home Care Coordinator, you'll be responsible for ensuring a reliable and responsive customer experience through effective scheduling, coordination, and service recovery. You'll own the daily service plan, manage rostering and shift optimisation, and lead backfill and surge responses. Your work will directly support our mission to provide timely, compassionate, and consistent care to clients in their homes and communities.
This role does not hold clinical governance. All clinical decisions and escalations remain under the Clinical Care Coordinator.
