Bendigo Health is seeking a Grade 2 Social Worker to join our Social Work Department, with a primary focus on supporting patients across our Subacute Inpatient Services, including Rehabilitation and Geriatrics.
This is an exciting opportunity to work within a multidisciplinary team and support patients and their families through often complex periods of illness, recovery, adjustment and discharge planning.
About the Role
Working primarily across the Subacute Wards, you will provide high-quality psychosocial assessment, intervention and support to patients and their families.
You will work closely with patients, families, carers and the multidisciplinary team to identify psychosocial needs and develop practical, person-centred strategies that support recovery, independence and safe discharge.
Key areas of practice include:
Providing psychosocial assessment and intervention to patients and their families.
Supporting patients experiencing adjustment to illness, disability, changing circumstances and loss of independence.
Assisting with complex discharge planning and transition from hospital to home, residential care or other appropriate supports.
Supporting families and carers to understand and navigate changes in care needs.
Identifying and responding to psychosocial, financial, housing, family and social support concerns.
Supporting patients experiencing grief, loss, anxiety, relationship difficulties and other psychosocial challenges.
Advocating for patients and promoting person-centred decision-making.
Facilitating communication between patients, families and the multidisciplinary team.
Connecting patients and families with appropriate community services and supports.
Contributing to multidisciplinary meetings, case conferences and care planning.
Providing Social Work support across the wider department when required, including responding to changing service demands.
About You
We are looking for a Social Worker who is committed to providing high-quality, patient-centred care and who enjoys working collaboratively within a multidisciplinary environment.
You will bring:
An approved qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
Demonstrated ability to undertake psychosocial assessment and provide appropriate Social Work intervention.
An understanding of the impact of illness, ageing, disability and changing functional capacity on individuals and families.
Strong communication, interpersonal and advocacy skills.
A commitment to reflective practice, professional development and evidence-informed Social Work practice.

Your New Division
The Community and Continuing Care Division provides a broad range of high-quality, person-centred care programs and services to consumers in inpatient, outpatient, and community settings. Each service within the division is designed to ensure holistic care and improve the overall well-being of our patients and communities.
The Community Services team is dedicated to enhancing the health and wellbeing outcomes of the communities in the Loddon Mallee with six regional offices. This team includes: Aged Care Assessment undertaken on behalf of My Aged Care; Community Allied Health; Community Care; Carer Support and Community Nursing & Home Care.
The Continuing Care team delivers high-quality services across the Loddon Mallee region including: Dental Care; Chronic Disease Management; Outpatient Rehabilitation; Support for People Transitioning Home; Diabetes Management and Geriatric Management and Assessment.
The Allied Health team provides comprehensive, high-quality care across the continuum, including expert services in: audiology, dietetics, exercise physiology, occupational therapy, physiotherapy, podiatry, psychology, social work, speech pathology and allied health assistants who work with these disciplines.
The Geriatric Medicine Team includes Geriatricians, Rehabilitation physicians, Palliative care physicians, registrars and junior medial staff. The Team work across inpatients, outpatients and home settings.
The Clinical Support Services team includes Medical Imaging incorporating BreastScreen, Pharmacy and Pathology.
In addition, the Community and Continuing Care Division holds the professional portfolio of Chief Allied Health Officer. The Chief Allied Health Officer and allied health discipline managers provide professional governance for all allied health across Bendigo Health.
