Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.
We are seeking applications from qualified Exercise Physiologists to provide best practice care to patients in the Transition Care Program (TCP). The role aims to improve physical outcomes and optimise independence for older adults as they transition to the community after an inpatient stay. This is a fantastic opportunity to experience the public health sector, as part of an experienced and collaborative multidisciplinary team (including care-coordinators, physiotherapists, occupational therapists and allied health assistants). This is a flexible role, working across both inpatient TCP and community TCP programs.
The successful applicant will possess strong clinical reasoning skills, and proven capacity to provide holistic, patient –centred exercise and health promotion interventions. A high level of enthusiasm for improving health outcomes for older adults is essential. The successful applicant will have access to high quality supervision, from an experienced Exercise Physiologist, and be supported to participate in ongoing professional development.
