About the role:
Western Health is seeking an experienced and compassionate clinician to join our Emergency Department Care Coordination (EDCC) Team on a 12-month fixed-term, full-time basis. This is an exciting opportunity to work within a dynamic, multidisciplinary service that supports some of the most vulnerable members of our community by delivering innovative, person-centred care within the Emergency Department.
As an Emergency Department Care Coordinator, you will play a key role in improving patient outcomes by reducing avoidable hospital admissions, facilitating safe and timely discharges, and coordinating care for patients with complex medical, psychological and social needs.
This role has a strong focus on the Frequent Presenter Portfolio (FPP), supporting patients who regularly attend Western Health Emergency Departments through the development of individualised, trauma-informed management plans that promote consistent, safe and effective care.
