About the Diabetes Education Service
The Diabetes Education Service delivers comprehensive inpatient and outpatient diabetes education across Austin Hospital, Heidelberg Repatriation Hospital, and Royal Talbot Hospital, in close collaboration with our community partners.
As a Diabetes Clinical Nurse Consultant, you will play a pivotal role in providing expert support, education, and management for patients living with diabetes and their families. You will deliver high-quality, evidence-based care to optimise clinical outcomes and promote patient self-management across both inpatient and outpatient settings.
About the role
Reporting to the Manager, Diabetes Education, this position sits within the Medical & Cancer Services Division and will be primarily based across the Heidelberg Repatriation Hospital and Austin Hospital sites. Some off-site travel may be required.
In this role, you will ensure that patients and their families receive comprehensive, timely, and effective diabetes education and support.
