An exciting opportunity exists for an experienced, dynamic and highly motivated Credentialed Diabetes Educator to join our Primary Health Team on a fixed term part time basis for 16 hours per fortnight to assist with parental leave cover.
You will be a Registered Nurse or Accredited Practising Dietitian (or appropriate allied health professional) with Credentialed Diabetes Educator (CDE) status or working towards the same. You will enjoy a varied workload within our small rural community including outpatients, inpatients and group programs, providing services at our main Alexandra site, satellite location at Marysville and community settings as required and coordinating ADH’s Diabetes Support Group.
You will follow diabetes self management principles and ensure that patients are encouraged to be partners in their health care planning, goal setting and decision making, and collaborate with multidisciplinary team members to achieve desired health outcomes for patients.
The successful candidate will have:
· Advanced clinical skills with experience in acute and/or community nursing or dietetics
· CDE status or commitment to working towards the same
· A completed Graduate Certificate in Diabetes Education or equivalent or currently undertaking a qualification
· Be an excellent communicator with the ability to liaise across a multi-disciplinary team
