What you'll do
- Assess and triage referrals to the service
- Bbe telephone liaison for both new and existing Community Services clients
- Act as a resource and role model to other staff
- Undertake rostering and daily staff management
- Review and update relevant policies for good governance
- Conduct Quality Activity Audits and completion of quality data reporting
- Undertake urgent home visits as service demands require
- Communicate and collaborate with other stakeholders
- Use your advanced clinical skills and knowledge and to the use of advanced care planning for residents, clients, and patients
- Contribute to the strategic direction of Community Services Programs

Ideally you will possess:
- APHRA Registration for a Registered Nurse, Division 1
- An approved Bachelor of Nursing or an approved qualification and/ or experience as a Registered Nurse, Division 1 to enable AHPRA registration to be maintained
- Demonstrated broad understanding of professional standards and issues in nursing and the health care system
- Demonstrated knowledge of relevant legislation including the Drugs, Poisons and Controlled Substances Act, Health Services Act, OH&S Act (Vic), and Aged Care Act (Cwlth)
- Understanding of the relevant Quality Standards in relation to the District Nursing and Palliative Care Services and the continuous improvement process
- Well-developed clinical, analytical and problem-solving skills
- The exercise of sound judgement and decision making needs to be demonstrated through providing examples from their previous work
- Proactive management of risk
- Intermediate to advanced computer skills (M365 including use of MS Teams and SharePoint) willing to learn software programs to implement care
