About the facility
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
About Community Nursing
Barwon Health Community Nursing is the largest non-metropolitan based service in Victoria. Community Nursing consists of three integrated teams based in the Eastern, Southern and Northern suburbs of Geelong
Our program supports consumers and carers in their home environment to achieve optimum level of health independence through the development and delivery of a high quality, cost effective, locally relevant Community Nursing service. We provide services under the following funding streams- Palliative Care (CPC), Post-Acute Care (PAC), Hospital in the Home (HITH), Home and Community Care (HACC), Support at Home (SAH), Commonwealth Home Supports Program (CHSP), Traffic Accident Commission (TAC), National Disability Insurance Scheme (NDIS), Transitional Care Program (TCP) and Private.
About the role
A unique opportunity has arisen for a highly skilled Registered Nurse to join a supportive and innovative Community Nursing service in a role that offers autonomy, variety and professional development.
This position will be a short term contract covering a period of parental leave between 30/3/26 and 27/4/27 working 56 hours per fortnight across day shift, afternoon shift, week-end shifts and after hours on call. The successful applicant may pick up additional shifts up to 76hrs per fortnight.
Relevant current/recent community or acute nursing skills and experience is essential to ensure comprehensive client assessments, care planning and delivery of consumer centred care can be successfully implemented by the successful candidate.
In this role you will be responsible for the provision of patient focused holistic nursing care to those assigned to you in order to achieve desired clinical outcomes. You will have highly developed interpersonal skills as well as sound communication skills both verbal and written.
The successful applicant will need to have current/recent acute or community nursing experience. Experience & knowledge in the areas of wound care, palliative care, HITH, community health care principles (Active Service Model & Social Model of Health) and continence management is highly desirable.
