Role description
Inglewood Health are seeking a Community Health Nurse to provide and coordinate the outpatient cardiopulmonary rehabilitation program. This includes the management of intake/referrals, pre and post assessment of participants, leadership of the group program and completion of the associated documentation, reporting and quality activities. The cardiopulmonary rehabilitation program is delivered in a cross-disciplinary team environment to ensure patient safety and optimal outcomes.
The position supports and educates clients diagnosed with a cardiac or pulmonary chronic illness to engage in cardiopulmonary rehabilitation and achieve improved health outcomes.
