About the facility
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
Subacute at Home is an inpatient bed substitution model including GEM and Rehab at Home that supports people with complex medical and rehabilitation needs. The team's office is based at the Barwon health Staff Hub and the position requires driving throughout the greater Geelong area.
About the role
To act as a clinical leader in GEM and Rehabilitation nursing, providing care, education, mentorship and clinical advice at an expert level to the nursing staff within the Subacute at Home service to enhance clinical practice.
To be the point of care escalation to support timely assessment and intervention by the relevant members of the multidisciplinary team. To support the triage and coordination of medical resources to ensure that patient care is optimised at all times.
The Clinical Nurse Consultant (CNC) will provide holistic nursing care to people who have complex care needs and ensure the broader Subacute at Home team’s care is supported by clinical oversight. Care will be provided in the consumer’s home and will also place a high emphasis on empowerment.
The CNC will also provide a clinical resource, clinical advisory/developmental role.
