About the role
South West Healthcare (SWH) is seeking an experienced and compassionate Clinical Nurse Consultant - Care of Older Adults (Residential In Reach) to join our Community Rehabilitation Service.
This role has a strong and primary focus on the Residential In Reach (RIR) service, delivering expert nursing care to residents of Residential Aged Care Facilities (RACFs) who are experiencing acute or sub‑acute clinical deterioration. The aim of the service is early intervention, hospital avoidance, and timely care in the resident's usual place of living, while also supporting older inpatients within SWH who are experiencing delirium, frailty, or cognitive impairment.
Working autonomously and collaboratively, you will partner closely with GPs, Hospital in the Home (HITH), emergency and inpatient teams, RACF staff, residents and families to ensure coordinated, person‑centred care and smooth transitions across settings.
