As a Clinical Case Coordinator in the Dialysis at Home team, you will play a key role in supporting people to manage homebased dialysis safely, confidently, and independently. You’ll work within a multidisciplinary team that delivers innovative, integrated, patient centred care across Melbourne’s West.

In this role, you will coordinate and deliver high-quality, evidence based nursing care for patients undertaking home dialysis. You will provide intake assessment, education, training and ongoing support to patients and their families, ensuring continuity of care and safe practice at home.
