South West Healthcare (SWH) is seeking a passionate and dedicated Aboriginal Liaison Officer - Chronic Disease to join our team and help improve the health outcomes of Aboriginal and Torres Strait Islander people living with chronic and complex conditions.

About the role
This important position supports clients by promoting early intervention, self-management, and culturally safe care pathways for those living with chronic illness. Working collaboratively with hospitals, community health, aged care, and Aboriginal Community Controlled Health Organisations (ACCHOs), you will play a vital role in improving care coordination and reducing unnecessary hospital visits.
