The Transition Care Program (TCP) supports older people to regain independence and optimise their functioning following a hospital stay. This short-term, goal-oriented and therapy-focused program provides a coordinated package of care, including low-intensity therapies such as physiotherapy and occupational therapy, alongside nursing support, social work, and personal care.
At Central Highlands Rural Health (CHRH), we proudly deliver Transition Care services on behalf of Grampians Health, Bendigo Health Services and the Victorian Department of Health. Our program spans multiple sites, with bed-based care provided at Creswick, Daylesford and Kyneton Hospitals, as well as Trentham Hostel, and community-based care delivered in clients' homes across the Macedon Ranges and Hepburn Shires.
We are seeking a motivated and experienced Registered Nurse to join our Care at Home team as a TCP Case Manager. This is a rewarding opportunity to play a key role in supporting older people to achieve meaningful outcomes and safely transition back to the community.
In this role, you will bring strong assessment, organisational and critical thinking skills, along with a passion for person-centred care. You will work collaboratively within a multidisciplinary team to coordinate care, support goal attainment, and deliver high-quality outcomes for clients.
The ideal candidate will have experience in Transition Care Programs, chronic and complex care management, Hospital in the Home, or a similar specialist program. Most importantly, you will demonstrate a commitment to excellence in patient care and a genuine desire to make a difference.

Details of Appointment:
Permanent part-time position - 0.5EFT (20 hours/week)
Position will predominatly be based at Kyneton Hospital, however may be required to work accross campuses/location based on operational needs.
