About the role:
At Western Health, we are committed to delivering the healthcare of the future while providing the best care for the people of the West. We are seeking an experienced and motivated Nurse or Allied Health Professional to join our Bed Based Transition Care Program (BB-TCP) at Williamstown Hospital.
The Case Manager role plays a pivotal role in supporting older people transitioning from hospital to the next stage of their care journey. Working collaboratively within a multidisciplinary team, you will coordinate complex care needs, facilitate discharge planning, and support consumers and their families to navigate community, aged care and residential care services.
This is an exciting opportunity for a senior clinician who is passionate about aged care, care coordination, patient advocacy and improving health outcomes for older people.
