CHRH is a proud, approved provider of the Commonwealth Home Care Package Program, delivering high-quality care management for older Australians living at home. This program will transition into the CHRH Home Support Program when Support at Home commences, 1 November 2025.
We're currently seeking a compassionate and experienced Care Partner / Clinical Care partner to join our dedicated team. This is a permanent role with flexible working hours and locations, offering up to 0.7 EFT.
About the Role
As a Care Partner / Clinical Care Partner, you'll empower older people to make informed decisions about the care and services they receive are aligned with their aged care single assessment and personal preferences. Your work will be key to enhancing participant health, wellbeing, and quality of life.

About CHRH:
Central Highlands Rural Health (CHRH) is a rural health organisation that operates five campuses across Hepburn Shire and the Macedon Ranges in the Central Highlands of Victoria in Australia, north and north-west of Melbourne. It was formed through the voluntary amalgamation of Hepburn Health Service and Kyneton District Health on the 30th November 2019. Each of our campuses retains the name of the township in which they are situated - Clunes Health, Creswick Health, Daylesford Health, Kyneton Health, and Trentham Health.
