About the Role
NCN Health is seeking a Case Manager to join our Support at Home Program team. The Care Partner supports older people to live safely and independently at home through person-centered care under the Support at Home Program. They coordinate services, explain the program, and help participants understand their goals, budgets, and support options. The role involves monitoring care plans and budgets, promoting independence, and addressing risks and changing needs. Working within the NCN Health team, the Care Partner ensures continuity of care, quality improvement, and regulatory compliance.
