The Position
The Care Partner/ Case Manager is responsible for coordinating and overseeing a high-quality, person-centred Support at Home service for a portfolio of older people living in the community.
The role acts as the key point of contact for clients, their families and representatives, ensuring that services are coordinated around the client's individual goals, needs, preferences and circumstances.
The Care Partner/ Case Manager develops and reviews care and support plans, coordinates services and providers, monitors client wellbeing and outcomes, identifies and manages risks, and advocates for clients to ensure they receive appropriate and timely support.
The role requires a strong understanding of ageing, independence, safeguarding, community-based care and the principles of consumer choice and control.
About the role:
This role provides case management and care coordination for Support at Home clients, ensuring services align with individual goals, needs and funding. Responsibilities include conducting assessments, developing and reviewing care plans, coordinating service providers, monitoring wellbeing and service delivery, and facilitating access to health and community supports.
The position also identifies and manages risks, responds to safeguarding concerns, advocates for client choice and independence, and supports clients to navigate aged care and community services. Working closely with clients, families and stakeholders, the role promotes positive outcomes through person-centred, culturally appropriate care.
In addition, the role monitors budgets and service utilisation, maintains accurate documentation, ensures compliance with legislative and organisational requirements, and contributes to quality improvement initiatives and continuous professional development.
