An opportunity exists for a fixed term, parental leave replacement role as Cardiac and Respiratory Community Nurse at Alexandra District Health.
Cardiac and Respiratory Community Nurse is responsible for coordinating the cardiac and pulmonary rehabilitation programs at ADH on either a Monday or Wednesday. The nurse works closely with the physiotherapist and other members of the primary health team to deliver client centered programs for people living with a chronic cardiac or respiratory condition. The programs aim to enable clients to manage their chronic condition to ensure optimal quality of life.
The nurse is required to conduct an initial nursing assessment of all clients referred to the service. During this assessment, initial questionnaires and testing is collected. The nurse will then determine the best method for the client’s rehabilitation program. Post program testing and follow up progress appointments are also conducted by the nurse and part of the role involves delivery education sessions to both cardiac and pulmonary groups. Ability to provide smoking cessation counselling and education is also desirable.
You will be a Registered Nurse with excellent digital skills including navigating databases, electronic client management systems and Microsoft Office. You will also have experience and interest in working with clients both individually and in groups and work well under pressure. We have a diverse client group and will be looking for someone who can interact positively with clients, staff and external health providers.
Flexibility exists to combine the role with 0.2 EFT Credentialed Diabetes Educator role which also requires parental leave cover – please see ADH website for details.
