About Us:
Located at Castlemaine, Loddon Prison is a 477-bed Medium-Security Prison for sentenced male prisoners. Adjacent to Loddon Prison is Middleton, a 236-bed Restricted Minimum-Security Prison for sentenced male prisoners. The focus of both prisons is rehabilitation via individual case management, employment, education, training and also the provision of programs and targeted interventions to address offending behaviour and promote the development of prosocial behaviour. Both prisons provide contemporary and quality correctional services that prepare prisoners for transition and reintegration.
About the Role:
The position of Assessment and Transition Coordinator is situated within the Offender Services Team at each prison. The Assessment and Transition Coordinator undertakes prisoner risk assessments using the approved CV assessment instrument, facilitates the parole application processes including liaison with the Adult Parole Board and Community Correctional Services and provides a range of support and services to assist prisoners to transition into, through and out of prison. This includes participating in the Case Management Review Meetings to develop and review progress against local plans; coordinating prisoner involvement in transitional programs; liaison and provision of assistance to external service providers; facilitating referrals, and delivering and coordinating the delivery of the programs.
The Assessment and Transition Coordinator is responsible for ensuring that the prison complies with requirements as outlined within the prison's Service Delivery Outcomes and other relevant policies
