Role description
Dhelkaya Health is seeking an experienced and dynamic leader to join our Senior Leadership team as the Ambulatory Care Manager to support leave backfill for a fixed term period. This key leadership role sits within the Continuing Care Directorate and is responsible for the strategic and operational management of a range of multidisciplinary services that support patients transitioning from hospital to outpatient and home-based care.
As Ambulatory Care Manager, you will lead the delivery of our outpatient rehabilitation programs, specialist clinics, and community-based exercise and wellbeing services. These programs are designed to restore function, promote independence, and support self-management of sub-acute and chronic conditions.
This role plays a vital part in ensuring the provision of safe, evidence-based, person-centred care that is accessible onsite, in the community, and through home outreach. You'll work collaboratively with internal teams and external stakeholders to coordinate care, optimise resources, and ensure a seamless patient journey.
We are looking for a leader with a strong background in health service delivery and a passion for value-based care. You will be committed to achieving excellent health outcomes, enhancing the consumer experience, and supporting a positive and engaged workforce.
