About the facility
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
About the role
As an assessment clinicians you'll work independently within a multidisciplinary team to deliver the Aged Care Assessment Program in the VIC 3 region. You're primary responsibilities will include conducting comprehensive assessments across physical, medical, psychological, cultural, and social care dimensions, and offering consumers appropriate service options. You'll collaborate with clients to assess their needs, goals, and preferences, focusing on wellness and reablement to promote independence. You'll provide information and refer clients to suitable services, including residential care, respite, community packaged care, and other relevant programs such as DVA, RAS, CHSP, Mental Health, or NDIS.
