About the role
South West Healthcare (SWH) is seeking a skilled and compassionate Advanced Developmental Paediatric Practitioner (ADPP) to join our multidisciplinary Pathways to Good Health (PTGH) team. This important role focuses on improving early identification and support for children in Out-of-Home Care (OoHC) who may be experiencing or are at risk of mild to moderate developmental delays.
The ADPP will play a vital role in delivering timely, community-based developmental assessments, ensuring children receive appropriate care at the earliest possible stage. Working within a child and family centred model, you will assess all key developmental domains including gross and fine motor skills, play, cognition, social development, and speech and language.
Using a comprehensive, needs-based approach, you will provide initial intervention strategies and carer guidance, while also identifying when more specialised discipline-specific assessments are required. This role ensures that children with complex needs are connected to the right supports through a collaborative, multidisciplinary model.
You will work alongside a dedicated team of professionals including paediatricians, nurses, psychologists, speech pathologists, and allied health clinicians. Outreach to community settings including schools, early learning centres, and residential care facilities is a key component of service delivery.
This position is part of a program currently funded until June 2027, offering a unique opportunity to support some of the most vulnerable children in the region and make a lasting impact on their health and developmental outcomes.
