South West Healthcare (SWH) is looking for a passionate and committed Aboriginal Liaison Officer to join our team and make a real impact on improving health outcomes for Aboriginal and Torres Strait Islander people living with chronic and complex conditions.
This is an exciting opportunity to be part of a role where your work truly matters — supporting individuals, families, and communities to access culturally safe, coordinated care and achieve better health outcomes.

As an Aboriginal Liaison Officer, you will play a vital role in helping clients navigate the healthcare system and access the services they need. You'll focus on early intervention, self-management, and culturally safe care pathways for people living with chronic illness.
This is a highly collaborative role where you will work closely with hospitals, community health, aged care services, and Aboriginal Community Controlled Health Organisations (ACCHOs) to strengthen partnerships and improve outcomes.
